Record Requests

Medical Record and Fire Report Requests

Fire incident and patient care reports can be requested by filling out the Medical Report/ Patient Care Request form or Fire Report Request form.
Note: If the medical report/ patient care report request you are requesting is for someone other than yourself, the Medical Waiver must be filled out by that person and also submitted.

There is a $12 fee for a copy of the requested record. Please pay by check made payable to the City of Santee.
